1. The Buyer/Member (herein referred to as Member) understands and authorizes LIFE KI-DO (hereinafter referred to as LK) to deduct any applicable registration fee and tuition payments directly from the above bank or credit card account according to the terms of this agreement. The Member authorizes LK to electronically deduct/process payments on or around the due date of each consecutive billing cycle according to the terms of this agreement. LK has the sole right to modify any payment due date.
2. Member understands that if any of their billing or account information changes, Member is responsible to notify LK in writing before the next billing period that will be affected and make arrangements to complete the necessary authorizations required to continue processing automatic billing.
3. Member agrees to have funds available at least 3 days prior to due date and will pay a service charge of $25 on any item presented for collection and returned for any reason. Accounts overdue are subject to additional collections department fees. LK reserves the right to cancel automatic  payment plan options.
4. The failure or inability of the Member to use the facilities, classes or services of LK for any reason, will neither relieve nor suspend the Member's obligation to make all payments required under this agreement on a timely basis, nor entitle the Member to a refund or credit of tuition.
5. Monthly memberships will continue until such time as LK receives a 30 day written notice of cancellation (email, phone or in person)
6. A late fee of $20.00 will be charged for any payment 10 days past due.
7. Upon notification, I authorize tuition rate increases.
8. LK is not responsible for any lost or stolen items.
9. By signing this agreement, the Member represents to LK that he/she has had an opportunity to observe and/or participate in the program selected by the Member prior to signing this agreement and that he/she is physically and mentally fit to take classes offered by LK in that program or any others selected by the Member.
10. If the Member's tuition/enrollment status changes LK will update this agreement accordingly.
11. The Member may cancel the agreement if the Member permanently moves more than 30 miles from the current location.
12. If LK or its facilities are substantially unavailable for use due to damage or loss by fire, accident, act of God or any other cause, the Member's  program will be extended for a period of time to equal the time of loss of availability, but no refund or credit will be due the Member.
13. The Member understands that classes may not be held on national or local holidays, scheduled breaks or other times as directed by LK.
14. The person(s) signing this agreement as a co-signer or co-buyer agrees to be jointly, severally and individually liable as if signing as the Member.
15. All rights and obligations of LK and Member under this agreement are subject to all applicable federal, state and local laws and regulations. When in conflict of this agreement, the contents of such laws and regulations shall be deemed to expressly modify this agreement and the agreement shall be deemed reworded to incorporate such text as may be necessary in order to make this agreement in compliance therewith. LK and Member agree to continue to be bound under the modified agreement including such text and further agree that no other modifications shall be deemed made to the agreement. If any portion of this agreement shall be deemed unenforceable, no other portion shall be unenforceable. Any waiver or delay by LK in enforcing any right under this agreement will not be a waiver or release thereof.
16. Upon acceptance as a Member at LK the Member agrees to faithfully comply with all provisions, terms and conditions hereof, including all rules and regulations of LK. LK reserves the right to immediately suspend or terminate any Member from participation or enjoyment of rights under this agreement for failure to comply with rules and regulations of LK. Suspension or termination of a Member shall not entitle the Member to a refund or credit for any tuition already paid. The Member shall be liable for any tuition due during the cancellation notice period.
17. Seminar/Tournament Fees and protective gear are not included in the price of this agreement.
18. Scheduling and content of classes and programs and furnishing of facilities and instructors and timing of such are at the sole discretion of AMAA. AMAA reserves the right to substitute or replace teachers.
